Below are the answers to some frequently asked questions about our new patient forms

Why do I need to complete these forms?

In order to serve you better, we need as much information as possible about your medical history and current condition. Additionally, basic demographic information is essential for our records.

I filled out the forms years ago; do I really need to complete them again?

If you’ve been a patient in the past, we will likely have your information on file, but it will need to be updated.

What if I don’t have all the required information?

We ask that you fill in as much information as possible. If there is missing information, we can assist you with that when you arrive for your appointment. But don’t wait to fill out the forms because you feel you are missing certain details.

Washington wellness is the best Chiro I’ve been too. Everyone is so friendly and the doctor actually listens and explains things to you. As a young woman who’s been spoken down to a lot in doctors offices I get nervous going- but not here. I notice a positive difference in my pain when I make it a point to come here. Bonus, there’s a red light sauna in the back that is incredible.

Dr. Swanekamp and his whole staff is just life changing! Everyone in the office is so kind and helping, knowledgeable and attentive- it’s an atmosphere you want to be in. My first visits to Dr. Swanekamp fixed things I didn’t even know needed fixing and gave me a whole new quality of life. This round, I came to him in more pain than I can remember. He got me right in and eased my pain naturally to get me through, and within a few days I was so much better. Even after initial healing they give you tools to continue to strengthen and heal so don’t end up the same way again. I can’t recommend this office and doctor enough.
