Why do I need to complete these forms? In order to serve you better, we need as much information as possible about your medical history and current condition. Additionally, basic demographic information is essential for our records.
I filled out the forms years ago; do I really need to complete them again? If you’ve been a patient in the past, we will likely have your information on file, but it will need to be updated.
What if I don’t have all the required information? We ask that you fill in as much information as possible. If there is missing information, we can assist you with that when you arrive for your appointment. But don’t wait to fill out the forms because you feel you are missing certain details.
